Please complete this application to request a new library card. Online applications receive provisional privileges; visit the library with valid identification to activate full access.

If you do not want to register online, you can print and fill out the application form, but to receive a card and library ID number, you must visit a library. Be prepared to show your ID and residence verification when you go. If you have lost or damaged your card, please report this to the library as soon as possible at (209) 754-6510. You can purchase a replacement card for $1.00 at any branch.
